Physical accidents are mental accidents, cosigned, aided and abetted by the emotional and spiritual.It doesn't matter whether it's the two space shuttles, the titanic, car crashes or plane crashes. Accidents have been discussed several times. All of these so-called accidents are due to broken logic due to humans making the error of knowing better but not doing better.

Though you mentioned lack of reporting and investigating, you have missed another opportunity to really bring out the fact that many incidents are not reported by pilots, flight attendants, ATC, ground crew, baggage handlers, engineers, refuelers, deicers, maintenance, parts manufacturer, system designers, program writers or others who are part of the flying process. And then there's the news media's selective focus. All partners in flying, including passengers should be able to report incidents and all should be able to check the status of post reporting followup. All incidents should be investigated, even pushback incidents. All partners should be trained to understand that an incident also includes any procedure not followed. Any type of incident can become part of the chain of events to more fatal incidents. This has been attested to by your videos. This is also attested to by the long list of obvious recommendations that should have already been known and implemented. There are no accidents, only incidents, most of which are caused by multiplicity of design, lack of pilot training, lack of pilot specialization, unimplemented recommendations, recommendations not turned into requirements, human incompetence and human behavioral tendencies or are unmitigated for the same reasons and the profit motive. Recommendations should become requirements and should be communicated to airlines, manufacturers, pilot unions and to the public. The public should know the risks they are taking and know when their expectations are out of line with decisions made by various partners. We expect everyone involved in the airline industry to operate airtight businesses in regards to safety.
The owner of the youtube channel has posted 3 videos since this one, but I didn't receive the notifications. Obviously the owner didn't like what I had to say and promptly canceled me from receiving notifications of future videos, while contradictorily leaving the comment as of last check on 06.01.23. This has happened many times because when I comment, I'm not concerned about lip service and obviously all those channels that have canceled my notifications are primarily concerned about getting people to watch so the channel can make money. This profit motive mentality permeates almost all human logic and behavior. I can still go to each channel and watch their videos, but why? The information benefit to me is far less than the monetary benefit to those channels.
